Thank you for your partnership with Discovery Falcon!
Partnering in the classroom is vital to the success of our curriculum which is based on project-based learning and small group developmental instruction. Our school relies on parents or other family members to partner in programs to make our vision a reality.
Families can help in a myriad of ways. Whether it is through helping a teacher in the classroom, leading an enrichment course, uplifting a student on the playground, or ensuring campus safety during drop-off and pick-up, our school relies on family partnerships to help the school run the way it does.
Families who choose Discovery are asked to partner in the following ways:
- Weekly Classroom Shift - Support your teacher for up to 2.5 hours per week (maximum 5 hours per family). This includes yard duty shift(s).
- Support 3 Field Trips - Drive or chaperone on at least 3 field trips.
- Community Work Day - Attend 1 Community Work Day. (Done outside of school hours)
- Support 1 Partnership Role - Roles vary in scope, interest, and time commitment, and many can be done outside of school hours. Parents are encouraged to choose roles that align with their own strengths and passions.

Download and complete the Live Scan Background Check Request Form.
Discovery hosts Live Scan events on campus several times throughout the year, please check school communications for upcoming Live Scan events. Families wishing to complete their background checks elsewhere may do so by visiting a Live Scan location(check list of locations here).
*Please Note:
- Your Live Scan background check needs to be done only once for Discovery.
- Live Scan / background checks from other organizations cannot be accepted.

Download and have your doctor complete the TB Risk Assessment Form
*Please Note:
- A skin/blood test or chest X-ray may be required if deemed necessary by your doctor
- TB risk assessment or test must be completed every 4 years
TB Testing Locations:
- Tuberculosis Skin Testing (TST) Sites / Chest X-Rays locations per Santa Clara County Office of Education

All field trip drivers must meet the following minimum insurance requirements:
- Minimum of $100,000/$300,000 bodily injury liability
- Minimum of $25,000 property damage liability
We understand that not every family is able to financially meet this requirement. In such cases, we ask families to chaperone on at minimum 3 field trips.

- ALL drivers, including those not intending to volunteering in the classroom, and extended family members, must complete all of steps 1-2-3 listed above.

Volunteer Check-In/Check-Out Procedures
All volunteers and visitors must check-in on the iPads in the school office prior to continuing onto the campus. When you leave the campus, please return to the office and check-out.
Checking in and out are vital to school safety. In case of an emergency, it allows us to account for all persons on the premises. If you plan to be on campus for any length of time, please check-in.
